Testing code request

Please fill out form to receive a code via EMAIL to book a covid-19 test by using the Aruba Health App. It can take a few minutes before receiving the code. If you do not receive a code within an hour, please try with another email address or check your email address settings.

It is important to note that the code will only work if you have an AZV number and general practitioner filled out and selected in your profile in the Aruba Health App. If you do not have an AZV number, please contact the Department of Public Health at 280 0101. PLEASE CHECK YOUR JUNK/SPAM FOLDER IN THE CASE THAT YOU DO NOT RECEIVE A CODE.

This system is only allowed for AZV patients who are symptomatic (according to the case definition). You will not be able to test for Covid-19 if you are not symptomatic and/or traveling outside of the country.

It is important to get tested for Covid-19 in case you have any symptoms that could indicate a possible infection with SARS-CoV-2, the virus that causes Covid-19. In case you have other symptoms that are not mentioned on this list, please contact your general practitioner (doctor di cas). This is form is not valid for fit-to-fly Covid-19 . YOU WILL NOT RECEIVE THE PDF DOCUMENT THAT YOU NEED TO SHOW IN CASE YOU ARE TRAVELING OUTSIDE OF ARUBA.
